PUBLIC NOTICE

AFFIDAVIT OF LOSS OF INVOICE/RECEIPT

Notice is hereby given that ROY N. OCONG, of legal age, Filipino, and Barangay Captain of Barangay San Enrique, Gandara, Samar, has executed an Affidavit of Loss of Invoice/Receipt concerning the loss of an official invoice/receipt booklet bearing Serial Numbers 2928701 to 2928750.

According to the affidavit, the said booklet was previously kept at the Barangay Hall under the custody of the Barangay Treasurer. Despite diligent efforts to locate the document, it could no longer be found and is considered lost beyond recovery. The affiant further declared that the booklet has not been confiscated nor turned over to any person or authority.
