PUBLIC NOTICE

AFFIDAVIT OF LOSS OF INVOICE/RECEIPT

Notice is hereby given that MARITESS N. PADANG, of legal age, Filipino, and Barangay Captain of Barangay Burabod I, Gandara, Samar, has executed an Affidavit of Loss of Invoice/Receipt concerning the loss of an official invoice/receipt booklet bearing Serial Numbers 0742251 to 0742350.

According to the affidavit, the said booklet was previously kept at the Barangay Hall under the custody of the Barangay Treasurer. Despite diligent efforts to locate the document, it could no longer be found and is considered lost beyond recovery. The affiant further declared that the booklet has not been confiscated nor turned over to any person or authority.
